"faithfully execute their duties"

There have been a number of reactions to the near-fatal attack on Donald Trump.

Here is mine.

Somebody had responsibility for

1. identifying all locations within both gunshot and eyesight range of the stage at Trump's rally, and 

2. ensuring that all such locations were secure while the event was ongoing.

Obviously those tasks were not successfully performed.

The question is:

Why?
